Welcome to the application process for the Connecticut Home Care Program for Elders.
 
The purpose of the Home Care Program For Elders is to give people an opportunity to live at home instead of going to a nursing home. People can find out whether they may qualify for any of the services from this program by completing the Request Form link Connecticut has provided. Persons who qualify for the program will also be required to complete a brief health screen. Once the health screen is completed, someone will contact you within 48 hours to review your screen.
